WorkAbility/Transition Partnership
Secondary Transition Services (For Ages 14–22)
“Transition services” for students with a disability are a coordinated set of activities that:
- are designed to be results-oriented, focused on improving the academic and functional achievement of the student, and facilitating their movement from school to post-school activities including postsecondary education, vocational education, integrated/supported employment, continuing/adult education, adult services, independent living, or community participation;
- are based on individual student needs, taking into account their strengths, preferences, and interests; and
- include instruction, related services, community experiences, development of employment and other post-school adult living objectives, and (if appropriate) acquisition of daily living skills and functional vocational evaluation.
Students are referred by neighboring districts to Sacramento County Office of Education (SCOE) programs, including North Area Community School, Palmiter Jr./Sr. High School, Hickey Jr./Sr. High School, Colley Jr./Sr. High School, and El Centro Jr./Sr. High School). Students may also be referred to programs for students with moderate to intensive support needs or young adult programs at various sites.
SCOE also supports Galt Joint Union High School District and River Delta Unified School District students (ages 14–22) who have an IEP and are served by their district special education department.

WorkAbility I
WorkAbility I is a nationally recognized “school-to-world” program that provides middle and high school students receiving special education services with comprehensive pre-employment training and employment placement and follow-up. The goal is to facilitate a successful transition from school to work, independent living, and/or post-secondary education or training. WorkAbility I is grant-funded by the California Department of Education (CDE).
Transition Partnership Program
The Transition Partnership Program (TPP) builds collaborations between local education agencies and the Department of Rehabilitation for the purpose of successfully transitioning high school students with disabilities into meaningful employment and/or post-secondary education. The Transition Partnership Program is grant-funded by the California Department of Rehabilitation (DOR).
